Role of ECMO therapy in cardiopulmonary regeneration and cognitive function during heart and lung failure in mice

Ventilation of patients with severe acute lung injury due to bacterialinfections is frequently not sufficient to reach adequate peripheraloxygenation. Therefore, extracorporeal membrane oxygenation(ECMO) has emerged in recent years as important therapy of patientswith respiratory insufficiency due to acute lung injury. Moreover,ECMO has also emerged as tool of intensive care for the treatment ofpatients with acute myocardial infarction. Despite its importance inintensive care, various aspects of ECMO therapy are still poorlycharacterized. Against this background, two institutes of pulmonaryinfection research and clinical and experimental surgery havecombined their expertise to develop a mouse model of veno-venousECMO support, which has been employed to characterize the effectof ECMO support on a healthy lung itself, as well as in models oftoxin- versus bacterial induced acute lung injury in mice. Based onresults gathered in the first funding period, in the second fundingperiod of TP 7, we aim to decipher clinically unresolved aspects ofECMO therapy in a mouse model of VV- and VA-ECMO. First, wewish to determine to what extent ECMO contributes to reduction ofventilation-dependent bacterial infections in patients with respiratoryinsufficiency. Second, the effect of ECMO therapy on cognitivefunctions will be examined in respective mouse models. Finally, weaim to clarify whether VA-ECMO therapy contributes to regenerationof or even worsens cardiogenic lung edema in mice with acutemyocardial infarction and secondary lung edema.
